Your First Visit

When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office. We do attempt to send these forms ahead of time, if time allows.

Your initial exam will last approximately two hours. We will provide a full mouth set of radiographs, chart all existing dental treatment and chart any proposed treatment along with a periodontal exam and suggestions to help you care for your teeth. At the end of this visit we will provide an outline of your proposed treatment plan.
